FORMER Woodside Petroleum executive Dave Agostini (pictured) has been appointed executive of the state government-Commonwealth inquiry into the Varanus Island gas explosion, along with Kym Bills of the Australian Transport Safety Bureau. The cost of the inquiry will be capped at $1 million.
